Heartigrams are graphical representations of hearts shaped by joining different letters, usually symbolizing the love shared by the people whose letters are joined.
In this case Lesley requested a heart made by the letters D, A and K which are the head letters of her son and of her nephew as well.
The extensions on the sides forming a bracelet contain myrtle leaves, which symbolize love.
